Normally I wouldn't recommend a book but...if you manage to go to the bookstore and pick up this really thin book called "The Now Habit", just reading the first 20-25 pages gave me some insight into why I procrastinate and self-sabotage and all that...and like legitimately, just picking up a few things here and there in the book actually helped.
I have some stuff coming up I am preparing for as well, and you are not alone my friend. If my mind wanders while working out or doing work, etc, I just change my thought patter and tell myself if it isn't my best, it's not worth doing. Focus on the process. I recently made the background to my computer a 5-point Goal Chart with stuff along the lines of "Give my priorites the concentration they deserve." "I am entitled to success." So on....
The difference between winning in life and being mediocre in life is often just the ability to push through the fog of bad emotions and take action anyway. Like we all feel lazy at times or distracted, it comes down to whether we can push through that despite the bad emotions or not and still do the right thing. This post is about girls, but it can apply to anything else as well, like school work. Charlie Sheen The other thing is to just eliminate the distractions... get the junk food out of the house, disconnect the television, etc.. it will take a little while to reprogram one's brain off the distractions but first they need to be out of the house. And like Eug suggested, picking up a good book on the subject can be very helpful.
